New Patient Forms:  Printing and filling the forms ahead of your appointment will save you time and allow us more to to talk. Here are the basic forms:

• Patient Information Form (or Intake Form)
• Consent
• Health History Form
• Hormone Inventory (Male or Female)

Insurance and Billing:

We have contracted with Aetna, and take all other insurance programs as a nonparticipating provider. Generally, we do not contract with HMOs. When insurance covergae is in question, we collect at the time of service, but will bill your insurance company for the visit. We are a MediCare provider, and take standard MediCal, and Family PACT. We also take TriCare Standard and TriCare for Life.
